On this page The Office for Early Childhood Development is partnering with family day care to deliver a new pilot program to help parents and caregivers access development checks for their children at family day care. If your child has already had a child development check using the ASQ-3 in the last 12-months with this pilot, you may be eligible for a second free virtual child development check with a trained screener at a time that is convenient for you.A child development check can show your child’s strengths and any needs they may have, which will help you understand how best to support them. It does not include a health check. Health checks need to be done by a medical professional such as a doctor or nurse. Your child can also have a development check or a health check at the Child and Family Health Service (CaFHS).Information collection and useIf your child has a virtual child development check, some of the information collected by family day care will be shared with the Office for Early Childhood Development. This will include things like your child’s postcode, gender, date of birth, and check result. Names will not be shared. This information is shared for monitoring and evaluation purposes.Reporting will never include anything that could identify your child. All information is kept confidential. The Office for Early Childhood Development will store any collected information securely. You do not need to consent to sharing this information to receive a child development check at family day care. Not consenting to share information will not impact your relationship with either family day care, the Department for Education, or the Office for Early Childhood Development, now or in the future. You can withdraw consent at any time by informing family day care, and your information will not be included in reports from that point on.The Office for Early Childhood Development is partnering with the Parenting Research Centre to conduct an evaluation of the pilot. The aim of the evaluation is to find out what it was like for families to access the child development check. This will help the Office for Early Childhood Development improve services for South Australian children. You will be invited to undertake a short survey and talk to the Parenting Research Centre about your experience using the service.Virtual platformTo support the security of your personal information and your child’s privacy, the online platform Coviu is used when delivering virtual child development checks. Coviu is compliant with the Australian standards for online security and encryption. You will be sent a secure, unique link for the virtual child development check prior to your appointment. You are asked to respect the privacy of your child and the copyrights of screening materials by agreeing not to make recordings of the check. Telecommunication, including virtual child development checks, is potentially vulnerable and limited by the security of the technology used.If you have any questions or concerns, you can contact:Data Sharing and Analytics team, Office for Early Childhood Development,DLEducationDataSharingandAnalysis@sa.gov.auFamily Day Care Child Development Check team, Education.FDCChildDevelopmentCheck@sa.gov.auParent or caregiver consent form Family detailsChild's first name * Child's last name * Child's date of birth * Day 12345678910111213141516171819202122232425262728293031 Month JanFebMarAprMayJunJulAugSepOctNovDec Year 2003200420052006200720082009201020112012201320142015201620172018201920202021202220232024 Was your child born prematurely? * -- Please Select --YesNo Parent or carer's first name * Parent or carer's last name * Parent or carer's phone number * Parent or carer's email * Parent or carer's postcode * Educator detailsEducator's name * Today's date * Day 12345678910111213141516171819202122232425262728293031 Month JanFebMarAprMayJunJulAugSepOctNovDec Year 19001901190219031904190519061907190819091910191119121913191419151916191719181919192019211922192319241925192619271928192919301931193219331934193519361937193819391940194119421943194419451946194719481949195019511952195319541955195619571958195919601961196219631964196519661967196819691970197119721973197419751976197719781979198019811982198319841985198619871988198919901991199219931994199519961997199819992000200120022003200420052006200720082009201020112012201320142015201620172018201920202021202220232024202520262027202820292030203120322033203420352036203720382039204020412042204320442045204620472048204920502051205220532054205520562057205820592060206120622063206420652066206720682069207020712072207320742075207620772078207920802081208220832084208520862087208820892090209120922093209420952096209720982099 Child development checklistI consent for my child to have a virtual child development check * -- Please Select --YesNo I consent for deidentified data to be shared with Office for Early Childhood Development for reporting and evaluation purposes * -- Please Select --YesNo My child has had a child development check using the ASQ-3 in the past 12-months with the family day care pilot * -- Please Select --YesNo My child’s previous child development check outcome was developmentally ‘on schedule’ or ‘provide learning activities and monitor’ * -- Please Select --YesNo I have access to the internet and either a mobile phone, laptop, or tablet with a camera * -- Please Select --YesNo I have a quiet, private space available with access to materials to undertake activities with the child for the screening * -- Please Select --YesNo I agree to have a virtual child development check via the online platform Coviu * -- Please Select --YesNo I require an interpreter * -- Please Select --YesNo If yes, please specify language: * I consent for information about my child’s development to be shared with my child’s educator to help their learning and development * -- Please Select --YesNo Acknowledgement 1.
